This is regarding the discussion we had in the last month on the issue from the transitional care scenario for allied health community. I strongly believe the proposed policy in this context will serve as an amicable solution to resolve that issue. As you are very much aware that the transitional care (Kohlbury et al.,2017) scenario do work much for the care needs that may take up short time to may extend for intense care needs like for 24 hours of nursing and assistance. In this regard, I would like to bring to your attention, two different aspects that are making up key issues in this regard. The first is the lack of accountability at the top management and second issue is about the lack of proper mechanism to facilitate better decisions at field level during the care process. These two issues are interrelated too. If there is better accountability in the top management, then obviously the administration will be better and they will encourage better decision at the field level. One particular complication seen in the field level is centralized information repository is not actually being distributed throughout the organization properly, the information management systems is not effective. In large organizations, where the decisions are decentralized, such lack of sharing of core information is contributing to the inefficiency in the medical care. The top management lethargy to improve the effectiveness of the system, by continuous monitoring, tracking effectiveness and updating the operations for better performance in terms of better care quality, more accurate treatments etc is another issue. In this regard the issue can be resolved by implementing the following policy in the organization.

There is need for audit of the information management systems for their accuracy as well as for efficiency in disseminating the information. Audit need to focus on patient information management systems, their access and operational features and the ways and means of their availability at different care locations of the organization need to be audited.
